While I was going through the A-merc brass, there were quite a few that were off center with flash holes as well.
Anecdotally, its generally because the tools they use to either ram or drill the flash holes are out of calibration, but I like the thought of some guy drinking on the job, smashing the drill press down on .223 brass willy nilly better.
